How we manage your personal information

Wellness Vision (“we”, “us”, “our”) provides its website and https://www.wellnessvision.com.au and/or any other domain names owned by Wellness Vision (“Website”) along with the professional services offered to you (“you, “your”, “User”) subject to this Privacy Policy and Terms and Conditions (“Agreement”).

We are committed to providing quality services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information.

We have adopted the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (the Privacy Act). The APPs govern the way in which we collect, use, disclose, store, secure and dispose of your Personal Information.

What is Personal Information and why do we collect it?

Personal Information is information or an opinion that identifies an individual. Examples of Personal Information we collect include: names, addresses, email addresses, phone and facsimile numbers.

This Personal Information is obtained in many ways including questionnaires, through our clinic software, intake forms, consultations, by telephone and facsimile, by email, via our website www.wellnessvision.com.au or from other publicly available sources and from third parties. We don’t guarantee website links or policy of authorised third parties.

We collect your Personal Information for the primary purpose of providing our services to you, providing information to our clients and marketing. We may also use your Personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. You may unsubscribe from our mailing/marketing lists at any time by contacting us in writing or clicking on the unsubscribe link at the bottom of the email.

When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

Sensitive Information

We understand that some personal information is particularly sensitive.

The sensitive information we ask you to provide for this purpose may include:

  • Date of birth; gender assigned at birth; gender identity; ethnicity; health and medical history (including any diagnosed or suspected medical conditions and current complaints); family history; known or suspected allergies, sensitivities or intolerances; current & past medications and supplements; occupation and past occupations; names and ages of children; marital status; if you are pregnant or breastfeeding; and details on treatments/services provided by other health professionals. This allows me to understand your health history and safely prescribe your natural medicines.
  • Information about your diet and lifestyle (including emotional/mental health), including current and past special diets, habits, stressors, concerns, preferences and aversions. This allows me to better understand your needs and to formulate a suitable and achievable treatment plan.
  • Pathology results and the results of any physical examinations that may be performed by us, including bioimpedance analysis (e.g. Metascan), blood pressure, pulse, heart rate, temperature, nail, tongue, skin, hair, teeth, grip strength, urine analysis and any anthropometric measurements taken. This allows me a further understanding of your health status and allows me to create your personal treatment plan (also referred to as your personal wellness plan).
  • Other information we may ask for includes, emergency contact details, the name and contact details of other health professionals you see.

We are committed to securely storing and handling your sensitive information.

Sensitive information will be used by us only:

  • For the primary purpose for which it was obtained
  • For a secondary purpose that is directly related to the primary purpose
  • With your consent; or where required or authorised by law.

Storage of sensitive information

Wellness Vision will take reasonable steps to protect personal information it holds from misuse, interference and loss, and from unauthorised access, modification or disclosure.

Our physical practice is located within Atune Health Centres (“ATUNE”), 52 Ada Street, Cardiff NSW 2285. We operate as a separate entity and utilise practice management software separate from Atune Health Centre from March 2024, however information prior to this date remains securely stored by Atune as part of their compliance. If you wish to enquire about the information retained by Atune you can email them at reception@atune.com.au. Additionally, after March 2024 if you wish to purchase supplements from their Acute Herbal Dispensary you will consent to Atune’s storage of your personal and sensitive information including your name, address, email, phone number, allergies, current medications and treatment plan. Programs we use to store your sensitive information
Some sensitive information may be stored securely online, or in the cloud through GoogleDrive. Information stored may include personal details, health history, consultation notes, tests results, prescription and treatment plans. You can find out more about Google Drive’s privacy policy here.

Your personal and sensitive information may be stored by Practice Better, Cliniko and/or Shinepages. Information stored may include personal details, health history, questionnaires, consultation notes, test results, prescriptions and treatment plans. We use a service called Mailchimp to send electronic newsletters to you via email. If you subscribe to this list, your name and email address are stored on Mailchimp servers to enable me to communicate with you in this way. You can opt out at any time by clicking the ‘unsubscribe’ button at the bottom of emails. Find out more about Mailchimp’s privacy policy.

If you are prescribed a product we may add our prescription to an online supply company. This allows you to place and order with this company and have the product sent directly to you. To do this these companies may store your name, home address, email address, telephone number, credit card number, prescription, order history, product preferences and current medications. Third Parties

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances, we may be provided with information by third parties (e.g. from your doctor or pathology centre). In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

Disclosure of Personal Information

Your Personal Information may be disclosed in a number of circumstances including the following:

  • Third parties only with your consent to the use or disclosure; and
  • Where required or authorised by law.

Security of Personal Information

Your Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorised access, modification or disclosure.

When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, Personal Information is required to be kept by us for a minimum of 7 years. After this time this information may be destroyed.

Collection of Information from Minors

Sensitive information may be collected from children under the age of 18 under the following circumstances:

  • in the presence of their parents or guardians, with their full consent
  • if they subscribe to my newsletter.

All information collected from minors is securely stored in accordance with this privacy policy.

Access to your Personal Information

You may access the Personal Information we hold about you and update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us at admin@wellnessvision.com.au.

Wellness Vision will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information.

In order to protect your Personal Information we will require identification from you and may require you to sign an Information Release Consent Form before releasing the requested information.

Maintaining the Quality of your Personal Information

It is important to us that your Personal Information is up to date. We will take reasonable steps to make sure that your Personal Information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you.

Use of medically compliant AI note taking

We may use Practice Better’s integrated AI Charting Assistant to take consultation notes so that we can be more present during our conversations. Their charting assistant is fully compliant with all relevant healthcare regulations, including HIPAA. Please view their responsible use of AI guidelines. If you do not want us to use the AI charting assistant during your consultation, please let us know and we can disable this feature.

This Policy may be updated from time to time to ensure continued compliance with the Australian Privacy Policies. Our current privacy policy is available on our website.
